How to use it
What this is
A symptom episode tracker in Excel and Google Sheets: one row per episode, automatic frequency tables for symptoms and triggers, a dashboard and a clinician report.
1. SETUP
Name, date of birth, tracker start date, existing diagnosis, medication, your main concern and your clinician.
2. SYMPTOM LOG
One row per episode. Same day, two different symptoms? Two rows. Record severity 1-10, duration in hours, a possible trigger from the dropdown, what helped and any notes.
3. DASHBOARD
Episodes logged, days covered, episodes per month, average severity, severe episodes, average and longest duration, % severe, and full frequency tables for symptoms and triggers with the top one named.
4. REPORT
Print the clinician report before an appointment.
Tip
Be consistent with symptom wording — use the same words each time so the frequency table groups them properly.
